چکیده

Spasticity is a common complication after spinal cord injury (SCI) within the upper motor neuron syndrome. We describe it in 2 parts: in the first one (Part 1) the pathophysiology, clinical features and assessment are shown, and in this part (Part 2) its treatment will be considered. Once it has been decided to treat spasticity, the treatment is based on starting with simpler and less problematic activities, and increasing in complexity the procedures against spasticity. The possibilities of therapy with its characteristics are described: control of triggering factors, positioning, physical modalities (stretching, muscle strengthening, cold, splinting/ orthoses, electrical stimulation, robotic devices), oral medication (baclofen, diazepam, clonazepam, tizanidine, clonidine, and cannabinoids), intrathecal baclofen, local injections (phenol, botulinum toxin) or surgical interventions. In order to assess the effectiveness of the different applied treatments we must measure outcomes. A management protocol is proposed for the spasticity of the SCI patient.

Comparison of Agonist vs. Antagonist Stimulation on Triceps Surae Spasticity in Spinal Cord

Objectives: One of the most common and disabling complications that affects individuals with spinal cord injury is spasticity. The purpose of this study is to compare the effect of agonist and antagonist electrical stimulations on triceps surae muscle spasticity in patients with spinal cord injury. Methods: A total of 30 subjects with spinal cord injury were considered for the study. They were...

Effects of pulsed radiofrequency on spasticity in patients with spinal cord injury: a report of two cases

Spasticity following spinal cord injury (SCI) results in functional deterioration and reduced quality of life. Herein, we report two SCI patients who presented with good response to pulsed radiofrequency (PRF) for the management of spasticity in the lower extremities. Patient 1 (a 47-year-old man) had complete thoracic cord injury and showed a phasic spasticity on the extensor of both knees (3-...

Global gene expression analysis of rodent motor neurons following spinal cord injury associates molecular mechanisms with development of postinjury spasticity.

Spinal cord injury leads to severe problems involving impaired motor, sensory, and autonomic functions. After spinal injury there is an initial phase of hyporeflexia followed by hyperreflexia, often referred to as spasticity. Previous studies have suggested a relationship between the reappearance of endogenous plateau potentials in motor neurons and the development of spasticity after spinaliza...

Thoracic 9 Spinal Transection-Induced Model of Muscle Spasticity in the Rat: A Systematic Electrophysiological and Histopathological Characterization

The development of spinal hyper-reflexia as part of the spasticity syndrome represents one of the major complications associated with chronic spinal traumatic injury (SCI). The primary mechanism leading to progressive appearance of muscle spasticity is multimodal and may include loss of descending inhibitory tone, alteration of segmental interneuron-mediated inhibition and/or increased reflex a...

Antispastic therapy with botulinum toxin type A in patients with traumatic spinal cord lesion

OBJECTIVES The purpose of this study was to determine the effect of botulinum toxin injections for the treatment of spasticity after traumatic spinal cord injury. METHODS 9 patients were included in this prospective designed study, with a follow-up of at least 2 years. All patients suffered from a massive spasticity after traumatic spinal cord lesion. Conservative treatment options did not sh...
